Curves Pinelands - Worst Service Ever!

I have been trying to cancel my contract with Curves Pinelands for the last month and a half and I am appalled at the terrible service I have received. I have left countless messages for the branch manager, Shantel, to contact me, and I have literally only received one call back from her. I have been nagging them for the cancellation form and only received it a month later from my first request, after I was informed by one of their staff members (conveying a message from the manager who wouldn't return my calls) that everything has been sorted and that there would be no need for me to complete the cancellation form because they have cancelled my contract - I have been requesting an email confirmation of this, but still have received nothing to date. I am disgusted at the poor service delivery and unprofessionalism at Curves Pinelands and would definitely NOT be recommending them to Anyone!!
