request for 17.4 document

please i requested Zanele and Thando to send me a copy of a 17.4 Document,please. since i been waiting cause you put me under Debt review and I am not happy..please send me the document as i need to clear my name. i cannot apply for anything due to this stupid Debt review that i never agreed to.<br> <br> this is messing up my chances when i apply for things like car,house,so please assist me to get this 17.4 document
