Policy cancellation

Hello my name is Clara Tshikororo. I have a complaint regarding Constantia. I dont remember opening a policy with this insurance company and I have never received policy documents but they are deducting money from my account every month. Where did they get such access and private information. I can't even reach them on the landline. I want this policy cancelled with immediate effect. Please!!!!!
