bad management

please help my name is Cindy Williams I was employed at CNA canal walk from November 2014 and resigned 3 weeks ago and filled in all the necessary forms and was told by management that my leave money would be paid the following week its been 3 weeks now and I have contacted HR today and they told me that the store did not submit my termination forms and they cannot pay me my leave money my employee number is *** last year I had the same problem with management when I went into premature labour at 6 months pregnant and I requested maternety UIF forms so I could provide for my baby till I go back to work and management refused to give me the forms saying that because I did not inform them I was pregnant they will not be gIving me any maternety UIF forms so I could not claim please help somebody
