Poor coordination of specialists and general lack communication to patient's loved ones

Extremely disturbed by the manner of admission, coordination and discharge. I am the next of kin but was never introduced to the patient's gp. The specialists were good but their work was uncoordinated. I got to know that my wife is discharged when I went for a visit. She was not even in a state of homecare. No referral or advise of how we should care for the patient. So pathetic. Sad because it represent black mediocrity.
