Senzo Mkhize *****ulently signed me up for a Clientele Funeral without my consent

I wish to formally lodge a complaint regarding a funeral policy that was *****ulently activated in my name without my consent.

I was approached by a clientele agent named Senzo Mkhize at Festival Mall, Kempton Park, who asked whether I was interested in taking out a funeral policy. I clearly and explicitly informed him that I was unemployed and did not have the financial means to afford or maintain such a policy. I further advised him that I already had funeral cover with another service provider.

After I refused to provide any additional information for a policy application, he then misrepresented the purpose for requesting my details by stating that he merely needed my ID number and cellphone number so that I could “rate his service” and assist him in earning R10. At no point did I consent to the opening of a funeral policy, nor did I sign any mandate authorizing a debit order or policy activation.

I was therefore shocked and distressed to discover that an amount of R350 had been debited from my account without my authorization. This constitutes an unauthorized debit order and *****ulent misrepresentation.

I hereby demand the following:

1. Immediate cancellation of the *****ulent funeral policy; 2. Immediate cancellation of the unauthorized debit order; 3. Full reimbur*****t of all amounts deducted from my account; 4. Written confirmation that the policy has been terminated and that no adverse records will be kept against my name.

I also wish to place on record that I intend escalating this matter to the relevant Ombudsman, the National Consumer Commission, and any other applicable regulatory authorities, as this conduct amounts to deceptive and ********* business practices. Consumers should not be misled into policies under false pretenses.

I trust this matter will receive urgent attention and be resolved without delay.

You requested to cancel the policy and the cancellation of the debit order process was confirmed.

As per our telephonic discussion, the policy was still within a Cooling off period and the refund process was explained to you by Rhandzu Manganyi.

Dear Sir/Madam,

I am writing to formally follow up on my previous complaint regarding the *****ulent activation of a funeral policy in my name without my consent and the unauthorized deductions from my bank account.
Despite previously lodging a formal complaint and sending multiple emails regarding this matter, I am disappointed that I have still not received my refund, nor have I received satisfactory resolution or feedback regarding the unauthorized deductions taken from my account.
As previously stated, I never consented to the activation of this funeral policy, nor did I authorize any debit orders or deductions from my bank account. The continued delay in resolving this matter and reimbursing the funds ********ly deducted is unacceptable.
I therefore once again demand the following as a matter of urgency:

• Immediate cancellation of any debit orders linked to the policy;
• Full reimbur*****t of all unauthorized deductions that have still not been refunded;
• Written confirmation that no adverse records, penalties, or negative implications remain against my name;
• A formal response explaining why this matter remains unresolved despite my previous complaints and follow-up emails.
Please treat this matter as urgent and provide written feedback within 48 hours of receipt of this email. Should this matter continue to remain unresolved, I will proceed with escalating my complaint to the relevant Ombudsman, regulatory authorities, consumer protection bodies, and any other applicable institutions.
I trust this matter will now receive the urgent attention it deserves.
