HOSPITAL CASH BACK PLAN

I was admitted to ICU on the 21st June 2016. Clientele Life was contacted about the claim and we were advised that I had to be discharged before we could claim. It has been 7 weeks and we are still waiting for the claim to be approved, We have sent documentation giving Clientele Life permission to obtain documents from the hospital, but it has now become our problem to get these documents. The hospital has now advised that these documents have been archived and can take up to 4 weeks to obtain. Waiting for Clientele Life to come back to us.

As per the telephonic discussions we had with the assessors and Mr T Sanders, on the 17 August and then again on 08 September 2016, he was advised that we are unable to proceed with the claim due to outstanding documentation. The assessors emailed him the requirements as requested on the email address that he provided.
Please note that the claim can only be assessed once all the relevant documents have been received.
We trust you find this in order. However, should you wish to contact us again please do not hesitate to do so.
