Poor Data Capturing

Clicks Key West failed to record my medicine script correctly. It's was a 3 months repeat and yesterday (05 July 2020) when I went to pick up my 2nd repeat I'm told that I've already collected 2 of 2 yet my repeat was for 3 months and I had only checked out 1 medication box in June, a Desarey Smith was responsible for the dispensering of my first collection (written on the medication label from 04 June 2020). I don't understand how and why would someone do such a thing by capturing false repeat times yet they have the script on them for reference. I would really want this to be taken care of ASAP! No way will I have to pay again at my GP for the same script twice.
