Operations management plus HR are inconsiderate

Operations management plus HR are inconsiderate. I am currently on maternity leave and have requested a UI19 form or confirmation of dates of employment as per labor requirement and i am still waiting for two months now for the correct information. Hr are not answering their calls and store manager is a prayer case with feedback. This is not on!. Bills need to be paid. Does this mean i was only good when i was used?
