Gas COC issued but not to standard

We had David come out and to do our gas COC as we were selling our house and he did the installation of our hob and gas braai. The COC was completed and issued which we then paid for them. A couple of weeks went by and we then requested the original COC to provide it to the new owners. David eventually supp**** us a hardcopy of COC which was issued in the incorrect name and it looked like it had been edited from a previous COC.

We had another service provider issue a COC they found that David had missed some issues that had to be rectified in order for the COC to be complaint.

I would strongly recommend not using David or Braai and Fireplace man for any services offered by them. They have been proven to be unreliable and I offered David the opportunity to rectify the problem but he cared not do so and continuously came up with poor excuses as to why it can't be done.

Also not that David works through a third party to issue COCs whom I contacted to query the state of the COC. The third party offered to come and perform a new COC but we had already used another supp**** to perform the COC as the house was about to transfer and further delays could not be afforded.

We advised the client right at the onset we did not currently have a COC engineer on our employ due to a death from Covert 19. However, we would put the client onto a third-party qualified gas engineer. we ask for any changes to the system and were only told of gas bottle being enclosed in a cupboard. we informed them it should be ventilated. They did this and sent us photographs.
The third party issued virtual copy of Coc certificate and client was happy. The conveyancing attorneys called for the original and they objected to an alteration on the original and insisted on a new certificate. the new certificate was couriered to Estate agent who had given incorrect or insufficient information for prompt delivery.
client then got another agency involved who found extra work had been done on their gas system, thus requiring an extra shut off valve. we not notified about this. 
As we were only the facilitator trying to help the client, we feel this complaint is uncalled for. 
Had we known the client had done extra work preceding our scope of work, we would have asked him to get the agency who did the retro work to provide the Coc certificate
