Worst service from Samantha

<p>I have never been so insulted in my life. I drove all the way from Florida to the Bonitas branch where I met a very rude Indian lady by the name of Samantha. Not only did she rudley tell me they no longe offer forms for chronic medicine but I asked for a card as the one I had was not reflecting surnames and the *********** woman decided to give me the exact same card to top it all off she helped another lady without even telling me we are done . It was after i gave her a stare where she asked. "Is there anything else you want to say" I felt so disrespected she treated me like I was on Bonitas for free. I am currently quoting for another medical aid with the hope of better service . I refuse to pay so much for my family and still get treated as such</p>
