To become a member are like pulling teeth

I've been trying since 12 December 2015 for my mother (a pensioner) to be accepted as a member of Bonitas. The application form was completed at Bonitas Florida on 12.12.15 and handed in and since then it has been an absolute nightmare with paperwork that either Bonitas lost or confused my mother with another applicant and switch their paperwork around. It has come to the point that my mother had to get new bank statements as Bonitas couldn't use the ones we mailed through in December. When mailing through the bank statements you have to use another mailing address, can't they mail it themselves to the new mailing addres?. Customer service??? Now today I get another mail asking for completion of section 9 and 11 of the application form, no application form was attached, never mind that the application form has been send to Bonitas about 4 times. Due to Bonitas incompetence and miscommunication, my mother will have to be on a 3 month waiting period, which are unacceptable. I need a solution ASAP and my mother on Bonitas medical aid as a matter of urgency.
