They don’t pay hospital bills

2024 claim still not resolved, they saying it’s a stale claim but there’s proof that the claim was submitted on time. I pay almost 10k monthly but now sitting with a 16k hospital bill. One moment they paid according to scheme rates the other no the 30/60 day rule was not followed when there’s clearly proof that they received the claim.
