Payment for medical services and treatment declined by Medical Aid

My general health practitioner referred me a specialist doctor with a referral note, which note I presented to the specialist doctor's secretary. Bonitas Medical Scheme paid for examination and tests, but declined the claim by the same doctor for consultation fee on the basis that I did not have a referral number. ( Rejected claim no:100321QH06N4 ) Naturally the specialist doctor has now invoiced me directly for that balance of the claim. Who is responsible for ensuring that a patient has both a referral note AND number before consultation?
