Incorrect tax certificate showing no contributions made

I have maintained uninterrupted medical aid payments with Bonitas (Boncap) for over six years, never missing a debit order. However, upon reviewing my tax certificate, I noticed that it only reflects contributions from January 2025, and it indicates zero contributions for the period of March 2024 through December 2024. This discrepancy is concerning as it will adversely affect my tax submission. I request that this error be corrected immediately and that a revised tax certificate, accurately reflecting all the contributions made, be issued without delay
