Income declaration forms

Hello Peter <br> <br> I really hope you will be able to help me. On the 3rd of March 2015 I received a nasty surprise when I went to the bank to withdraw some money. My bank account had a balance of R0.00. After I got a bank statement I saw two different amounts has been deducted by Bonitas. Upon enquiry I was told the R968 that was deducted was for being in arrears for not submitting my income declaration forms. I never received those forms. The person I spoke to emailed me the forms and after a week of phone calls to ensure they received all the necessary documents I was told that my premium was R675 and that my account had been credited. I have a debit order which is deducted the first of every month and up to date that amount has not been deducted which leaves me to believe that they merely subtracted it from the R968. Although I was told that they do not I want the R968 to be paid back to me. I had to rely on the generosity of others to be able to survive financially last month. I would appreciate it more if they paid back my money than a credited account. I sincerely hope you will be able to help me as R968 is a lot for me to lose.<br> <br> Thank you<br> Davida Smit
