Clinical review pathetic

<p>My wife has a pmb condition hyperthoidism. For this condition a couple of test are need to make sure the function of the throid is correct. The dr has select to do a tsh and t4 test together on the lab slip. You have failed the claim as this is not on the care plan. As per the clinical review these must be done seperately. It does not state in the CMScript issue 1 of 2014 that this must be done seperately. If the dr needs both test why would they tick 2 block where one block does both of them. As code 4484 is not covered on the care plan it is not paid. but this code is testing for tsh and t4 level and as stated by CMS this must be covered.</p> <p> </p> <p>Please advise if i need to take this to CMS or will you reprocess the claim for the 01/08/2016 and 31/10/2016. </p> <p> </p> <p>Regards</p> <p>Wayne</p>
