Bennifits/Rules 2018

This is the email that I sent I would like to confirm the following that on Page 33 of the 2018 contribution changes and benefit enhancements (Attatched) under CHRONIC BENEFITS you state and I quote.

"If you choose to use medicine that is not on the formulary, you will have to pay a 40% co-payment. You can get your medicine from any pharmacy on our network".

Does this then imply that as of January 2018 you will pay 60% for my FORVENT?

I have now sent this to them 3 times and still they refuse to comply but instead they have all sorts of conditions that don't even appear in there Rules that I am to meet, *******. I have sent them my final email today giving them 5 days to comply with there rules if not I'm going to Legal Wise to sort this out Regards Robert Gamage M.A. ********** 7956
