Lost cellphone.

I have help my phone in the Uber, I have been trying to get it back but the driver has changed the information to make it as if I never requested. They keep on asking me to supply them with more information which is a questionnaire asking if I was the driver. How can I be the driver if I was the one who asked for a ride.
