cancelletaion of policy

hi there<br> i have called to cancel my policy almost 3 weeks , some girl called saying she will send me cancelletion form on the 09th of december 2015 for me to complete . to date i have not received those forms , i called again and some girl said she will forward my message to the relevant department . now i want it to be known , i do not have a policy with you guys . whether or not i havent completed the forms , that is not my problem your staff didnt send as promised . i want nothing debited from my account from you guys .
