zero support to resolve a claim

Bestmed repeatedly rejected a claim made by Lancet, which was a valid claim. I enquired numerous times and I was told today that the referral letter is required. I need to ask for a copy and send it to Bestmed. 3 months later I have not moved forward and going in circles with no support or clear communication from Bestmed.
