Claims not being processed

My claims for medicine dispensed are not being processed (As part of Bankmed)<br> <br> The following are not being paid and your excuses are ridiculous:<br> <br> T-cream - a bio-identical testosterone cream - levels were test through reputable lab and found to be so low they couldn't test for it - Dr prescribed T-Cream as a measure to correct. We both agreed this the best option as it is a bio-identical cream and I prefer that above to the generally prescribed injections or tablets. (So does Bankmed not cover HRT?)<br> <br> DHEA: it is a precursor hormone to Testosterone and will also assist with my very low levels of Vit D<br> <br> Vit D: As per lab tests my levels are exceptionally low - Dr prescribed vit D3 to correct urgently - not being paid. Why? You can't tell the Dr that 120 tablets are too many - if that's what is required to correct my levels, then that is his medical decision.<br> <br> In your response letter to me you claim exclusion categories 19 and 98 - What does that mean?
