Dear Complaints Department, I am lodging a formal complaint regarding the handling of my insurance premiums and the debit orders processed against my bank accounts. I am extremely concerned and frustrated by what has happened. Three separate debit orders were taken from two different bank accounts, yet I have now received a notification stating that there was a short payment and that I do not have cover. I need a full explanation as to why this has happened. If these debit orders were intended to cover the new month's premium, then why were they processed earlier than the expected premium collection date? I need you to confirm exactly which month and which premium each of the three debit orders was allocated to. Please provide me with a complete breakdown showing: The date and amount of each debit order that was processed. Which bank account each debit order was taken from. Which policy/premium period each payment was allocated to. Whether any of the payments were allocated towards arrears or a previous month's premium. The amount you claim is still outstanding and why. The date on which you say my cover was affected or cancelled. The exact reason I received a short-payment notification despite multiple debit orders being successfully deducted. Confirmation of whether my policy is currently active and whether I have full cover. I also want you to explain why payments were taken from two different bank accounts if there was still supposedly an outstanding amount. If the debit orders were collected earlier than the agreed premium collection date, I would like an explanation of why this occurred and how the payments were allocated. I do not believe it is reasonable for me to be told that I have no cover when money has been deducted from my accounts on multiple occasions without a clear explanation of how those payments were app****. Please treat this as a formal complaint and not merely a general policy query. I require a written response and a full reconciliation of my account. I also request that my cover status be reviewed urgently, and that you confirm in writing that my policy will not be prejudiced while this payment dispute is being investigated, particularly if the short payment resulted from an error in the processing or allocation of the debit orders. If you maintain that I do not have cover, please provide the contractual and payment records supporting that decision, including the dates and amounts of all attempted and successful collections. I expect this matter to be investigated properly and escalated to the appropriate complaints department if it cannot be resolved immediately.
