I ended at the hospital after eating at Altitude Bar

I had a really bad experience at altitude bar. Which is located at Times Square in PTA. They gave our tablet away although we made a reservation and we arrived on time. The cutlery was very dirty. We had to send it back. They took our orders but we didn't get what we asked for. My girlfriend is at the hospital because they gave a meal that wasn't cooked. I have sever allergy reactions because they gave me avocado after I told them not to. Something needs to be done about this.
