Unable to provide relavent documents

Since 2016 I have been trying assisting my family friend to claim provident fund from Alexandra Forbes but they send me from pillar to pillar. Firstly they where saying to me member not on their list then later managed to pick him up. I was asked to send authority letter which we did. Now Mr Jack Mmako asked me to contact Group five for withdrawal form but group five it says its Alexandra Forbes that need to provide me with that form. Now i have no idea where will I get this form from as Alexandra Forbes(Jack Mmako, Innocentia Mtshali and Terry-Anne Scheepers) are aware that Group five( Jack Ntuli) says its Alexandra Forbes that need to send out the form. No idea what must i do or go to to get the relevant form.
