Terrible service

My request was very simple, I need to claim my provident fund due to retrenchment so please send me the necessary forms.

On the 16th of January Surprise Methi responded to my mail saying hes not allowed to send members their statements, the employer has to request it on their behalf. I didnt ask for a statement, I'm currently unemployed.

When I reiterated that I need a claim form he referred my matter to Karabo Magolo who then responded by saying that the funds had been transferred to a AFICA account, again, this is not what I asked for I need the funds transferred to my bank account. Once more my matter was referred to another AF department, mind you they both don't answer phone calls so how long am I expected to go back and forth with AF staff before my very simple request is processed?
