Really Confused

<p>I recently resigned from my job and I received the provident fund withdrawal forms from my previous employer to fill in and send back. I received the forms and they were submitted to Alexandra Forbes by the employer on the 6th April 2017. I then followed up with AF to check if they have received the documents but I was told that they haven't been received. I followed up with the employer and they are adamant that they have submitted the forms to Alexandra Forbes. I am now really confused and I don't know what is the correct route to follow from hereon since my previous employer insists that they have submitted the forms.</p>
