No provident fund pay out

I got retrenched over a year ago and submitted numerous withdrawal forms for my provident fund and still have not received any payment. When I did my sars tax return according to sars the money has been paid out and I had to pay tax on the funds that I haven't even received. After contacting Alexander Forbes they said it has not been paid out yet and they cannot help me.
