Help with cancelling Chartis Life cover!!!!

Please let me know how I can assist my mom cancel a Chartis Life insurance cover she had taken for her relative who passed on more than 2 years ago. To this day the debit order goes religiously through her bank account monthly and the banks say they can't stop it & she has to contact service provider.
