Unpaid policy claim

my cousin (Nokwayiyo Ga***i) joined Affinity funerals at the Grahamstown branch. she put me down as a beneficiary as I have been taking care of her. She didn't put down the correct name as I am popularly known by my nickname. The agent did not even request a date of birth of the beneficiary or an identity number of the beneficiary. She unfortunately passed away last in November 17, 2021. As I was looking through her documents to prepare for the funeral, I found out that she has this policy from affinity . I was told at the offices at Affinity in Grahamstown that it will be impossible for me to claim as she didn't put down the correct names and there is no date of birth under the beneficiary. The master's court couldn't give me a letter of authority because there was a beneficiary on the policy. I had to take out a loan for the funeral because I was her care giver. I find that affinity funerals was negligent on the matter as their systems shouldn't have accepted a policy application without an identity number/ date of birth. The agent that helped my cousin should have made means of getting in contact with me to confirm my details. She has been paying the policy since 2020 and +- R480 was debited every month without fail but when it came to the claim they did not take the matter seriously.
