unsolicited debits

according to my knowledge is no notice month required from insurance providers . Further should no settlement fee be demanded because no goods where supp****, but however contrary to the above am I being charged a settlement fee of R560 which also goes against their promise of unlimited doctors visits as per their day to day plan, not only is this wrong but its also ******** as I paid my December premium in full
