We have send you this dd 4 June 2018 with no reply.

We have paid all medical expenses out of our own pocket , because of the lack of service from Affinity.

Your urgent feedback is required today , I need confirmation as requested as the doctors advise the S J Anderson is not noted on the system as a member.

You advise last time that you will send it through to a department to add, this is still not done and I am very concern.

Please advise before 12h00.
