DOUBLE DEBIT AND UNKNOWN SUSPENSION
This past month I have been communicating with Affinity agents about a payment issue in December. On Tuesday the 26 January 2016 I went to the doctor only to find out my policy has been suspended since the 4th. NO ONE INFORMED ME. (*******) I emailed Affinity whom responded that it was in fact an agent who did not allocate my payment. As a result I had to pay the doctors (myself) and then on 2 February they double debited my account once again without my knowledge which resulted in financial difficulty as my account went in arrears and other debit orders were declined. I want to inform the public to never use AFFINITY HEALTH INSURANCE. I expect a full refund in the next 72 Hours for any inconvenience caused as well as bank charges for transactions that failed. By now Affinity owes me R2107.00.

This is the reason that the payment was not allocated to your policy at the time, as we could not trace the payment. Payments via EFT are traced via your policy number.
This also explains why a double debit was automatically loaded for February, as the January premium payment was reflecting as outstanding. This is also the reason that your policy was inactive at the time that you consulted your doctor.
We can confirm that the additional premium will be refunded to you. You are welcome to submit the doctor's account with proof of payment and a reimbur*****t form to us and we will gladly process your claim for reimbur*****t.
Kindly note that our Escalations Department has attempted to call you to discuss this in more detail.
