Good day Wilma,

Thank you for reporting your experience with us.

Please note that we have conducted an investigation and can confirm that you were advised by a consultant on 02-06-17 that we have no record of a double deduction running off of your account, furthermore, the aforementioned consultant requested that you produce a bank statement in the event that you were double debited in order for us to submit this to our Finance Department for further investigation.

Furthermore, we can confirm that due to the fact that your policy incepted on 01-05-15 your welcome pack containing all the relevant policy documentation was couriered to you on 24-06-15 and according to our records is in your possession.

In the event that you would like for us to further investigate your double deduction claim or if you would like for us to provide you with a new welcome pack please email us on *** and our Complaints Officer will gladly assist you.

Kind Regards,

Affinity Health Management Team

Nobody from Affinity contacted me on 02.06.2017.

You clearly do not even know which documents I requested. I am not in need of my Welcome Pack, I think I rather need a Goodbye Pack.

To my surprise one of your "consultants" did contact me a few minutes ago wanting to discuss my complaint. When I informed her that I am currently busy and she can contact me at 1, she said that she will not be able and that she can contact me at 1:30. After I explained why I have given a certain time she rudely explained that another "consultant" will contact me at 1.

Please let the "consultant" have her or his facts correct when they contact me.

As aforementioned should you wish for us to review this matter please provide us with your bank statement so that we can submit it to our Finance Department for further investigation. Please also advise on what documentation you have requested and we will ensure that it provided to you by close of business today as we currently have no record of a documentation request on our system.
