STOP BILLING ME - INSURANCE CANCELATION

I requested a cancelation of insurance in November but it's not canceled and I am still being billed??!!! I received your confirmation SMS last year saying my insurance is cancelation but it's NOT! PLEASE CANCEL AS I NO LONGER NEED IT AND SEND ME WHAT I OWE.
