PCR test to leave SA to ITALY that does not require PCR! Why?

We planning to travel to Italy, with Qatar airlines both only need proof of my vaccination certificate. OR tambo health department wants me to produce a PCR test to leave the country? On their own website it states that it depends on the country and airlines if you need PCR or not. How can they make their own rules?
