who has the damned cheek to forge a signature ????

Absa credit life insurance - i am been told that on the 27th of May 2020 i signed a form of cancellation of the above policy and i can tell you i never recieved and never signed any form of cancellation for this policy someone has written on this cancellation themselves got my phone number wrong and forged my signature - i never recieved or sent this form and i am very annoyed that someone within a banking sector has the damned cheek to write out this form and tell me that i sent this in to them - told the debt review on the 8th of may that i am now permanently disabled so why would i sign any form of cancellation of a credit life insurance if i am going through a fight to try and get debt cancelled because i no longer work and cannot afford payments
