Requests ignored!!!

On the 11 May 2015 I cancelled the request for a Credit Card. I paid the R147.86 cancellation fee (which is recorded on the account I have just received). I have been into the Bank twice again to cancel this card and also telephoned the bank - but it seems NO ONE IS LISTENING! I am now being charged R5.48 for a card that I HAVE NEVER HAD OR RECEIVED!!!!!! Please reverse this charge IMMEDIATELY
