Error Message

Been trying to apply for a funeral cover online but every time I do that I get an error saying \relationship status error"or something like that. I sent a request for one of their consultant to help with the error or application of which they did but only to be requested to re apply again. I did reapply only to get an error again. Went to the Rosebank branch and advised on my errors and the lady promised to email me funeral cover application forms but I haven't received anything.
