I would like to lodge a formal complaint regarding the unauthorized and *****ulent enrollment into a life insurance policy. The individuals concerned are elderly pensioners who were misled into believing that they were signing up for a policy. No signures were done however money is debited on both my pensioned parents.

The deductions from their pension accounts were only discovered after unauthorized premium payments began.

- Cancellation of the unauthorized policy with no penalties.
- Full refund of all premiums deducted, including any fees.
- Written confirmation that no further deductions will occur.
- An investigation into the conduct of the agent(s) or staff involved.
- Disciplinary action against any individual found responsible for the misconduct.
