*Subject:* Unauthorized Deductions from Salary - Policy No : BBSEP *** Not Consented To - Urgent Cancellation and Refund .
To Whom It May Concern,
I am writing to formally lodge a complaint regarding unauthorized deductions by 1Life Insurance from my salary.
In early February 2026, a 1Life broker visited my office. He requested to check whether I qualify for 1Life insurance. Although I showed no interest, he insisted he would only check my eligibility and that no policy would be taken if I did not agree. I provided my details solely for an eligibility check. The broker then informed me that I do not qualify.
To my surprise, two months later I noticed on my payslip that 1Life Insurance is debiting an amount of R 657.50 from my salary monthly.
When I contacted 1Life telephonically to query this, the consultant advised me that there is no policy under my ID number. Despite this, the deductions continue to reflect on my payslip. I have attempted to contact the broker who did the initial check, but he is not returning my calls. I have also tried to cancel telephonically, but the calls are ended while I am still speaking.

Furthermore, I have been contacted by individuals identifying themselves as 1Life representatives who have made unsubstantiated accusations regarding my consent to the policy. These claims are unfounded, as these individuals were not present during the policy's inception.
Despite several months having passed, the refund remains outstanding. Additionally, I have experienced discourteous behavior from your staff, including instances where calls were abruptly terminated while I was still speaking.
