Lost yellow form from car sale - complete incompetence

Absolutely pathetic and appalling service. They lost the yellow form that was submitted from the sale of my partner's car, and because the system was down on the day, he doesn't have proof of the exchange (although at the time was assured that the car had been taken out of his name). Now he's sitting with a car in his name that he doesn't even own anymore, and fees for the license which he had to pay in order to get the license for his new car. Plus he got accused of being a **** by the tellers at the Registration office and even had the one teller shut her window in his face. Shocking customer service.
