Cancelled account re activated without my consent

I cancelled my DSTV subscription in October 2011. I have now received a letter form your debit collectors for an unpaid account balance. On logging on the DSTV site it seems DSTV reactivated my account on the 31 October 2011 without my permission. My decoder has never been reconnected since my disconnection request.

A) no email, phone call, mail or SMS were sent to me on the reactivation
B) no email, phone call, mail or SMS were sent to me on the outstanding balance

I checked the details the DSTV website, and they are correct and unchanged.

Please would you reverse the charges to my account and permanently cancel my account. I am extremely annoyed with Multichoice for firstly reactivating my account without my consent and secondly never attempting to contact me as the account went overdue.
