I phoned the Labor Department in January to get the correct forms needed for maternity leave. The person I spoke to was friendly and promise to e-mail me the correct forms but I received them!! I then phoned again explaining that the lady I had spoken to the day before promised to e-mail the forms, and she said she will e-mail them immediately. I never received them. I am running out of time and desperately need the forms!!!!! |
