Knock Off duty before time without completing job |

On the 01/02/2012 we visited Home Affairs in Bloemfontein to enquire about the hand written death Certificate for my brother. The answer we received was that his ID was a duplicate, that's why is written in hand. The Officer in Bloemfontein phoned the Home Affairs in Betlhehem, as the post mortem was conducted in Bethlehem to find out, why didn't they give us a letter that needs to be attached to the Death Certificate, and ask them to write one, and, fax it to her so that she can hand it over to us. Mrs Ntsukuna from Bloemfontein spoke to Mr Lekgashe in Bethlehem Home Affairs who confirmed to her that he will track out the file, and write the letter and fax it quickly. We waited from 14H17 to 15H00 to receive the letter and we didn't. At 15H10 we approached Mrs Ntsukuna who phoned again for Mr Lekgashe, and find out that Mr Lekgashe knocked off duty without writing the letter as requested and promised. I ask department to discipline Mr Lekgashe for ignoring the client's request, as the letter was requested by the insurance, so that we can bury our brother. Mrs Ntsukuna helped us by taking the issue with the Regional Representative, and we got the letter.
